 All patient registration information is to be directly entered using our PATIENT PORTAL.  If you do not have a login ID/Username for our portal, please email us at Admin@HittingMilestones.com or call (732) 851-6947.

To maximize your time with the Therapist and professional staff, we ask that you review and complete our New Patient Forms on our Patient Portal 48 hours in advance of your visit.

Non-Sharing Clause: We do not share your information with third parties for marketing. Information is shared only with our SMS provider for the secure delivery of messages.
